E.T.D. – Emergency Travel Document
If the passport or travel document of an Italian or EU citizen is lost, stolen, or destroyed, the Consular Office can issue an EU Emergency Travel Document (EU ETD). This temporary travel document is valid for the duration of the trip for which it was issued, limited to the time strictly necessary for re-entry into Italy, the Country of AIRE residence, or the Member State of which the applicant is a citizen. For issuance, the applicant (including infants and minors) must be present at the Embassy.
The beneficiary of an EU ETD is required to return the document, whether expired or not, upon arrival at the destination to the border police or another police station, or to the consular office with territorial jurisdiction for the district of residence in the destination Country.
REQUIRED DOCUMENTATION
- EU ETD application form, correctly completed and printed;
- Declaration of theft or loss of the passport or other travel document made to the Consular office;
- Report of theft, loss, or destruction of the passport or other travel document to the local police authorities;
- Printed copy of identity card, passport, or driving license. In the absence of any identification document, the presence of a witness, holding an identity document, who is able to identify the applicant under his or her responsibility, is required;
- Recent ICAO-compliant passport photo;
- Travel document.
REQUIRED DOCUMENTATION FOR MINORS:
- Completed and printed EU ETD application form (both parents must sign);
° If the father has not acknowledged the child’s birth, a copy of the birth certificate showing the father’s name is missing, or another certification issued by the Municipality certifying the father’s absence, must be presented;
° If one of the parents is deceased, a copy of the death certificate or another certification issued by the Municipality certifying the death must be presented;
° If one of the parents is not a European citizen, they must authenticate their signature by going to the Embassy with their original passport;
- Original or copy of both parents’ passports or identity cards;
- Declaration of theft or loss of the passport or other travel document made to the Consular Office;
- Report of theft, loss, or destruction of the passport or other travel document to the local police authorities;
- Printed copy of identity card, passport, or driving license. If no identification document is available, a witness must be present, also holding an identity document, who can identify the applicant under his or her responsibility;
- Recent ICAO-compliant passport photo;
- Travel document.
The EU ETD is issued free of charge, except for reimbursement of the value of the printed booklet only.
